In 1955, the Harold M. Cosgrove award was established to recognize outstanding service by an individual to District II, now known as the Southwest Forum. Named for Harold M. Cosgrove of Tulsa, this award represents the highest honor the SWF has to offer. It's appreciative recognition of individual accomplishments spotlights years of dedication and exemplary service. HAROLD M. COSGROVE OUTSTANDING SUPPLY MANAGEMENT AWARD Contents: • Instructions for Nomination of Candidates: 1. Nominations must be received no later than August 24, 2007. 2. Nominations received after that date will not be accepted. 3. All information will be held confidential within the Cosgrove Award Selection Committee. 4. Nomination Requirements: a. Any member within the Southwest Forum may be nominated by their affiliate, another member of the Forum or by themselves. b. The member to be nominated must be in good standing with the Institute for Supply Management and the Southwest Forum. c. The current Forum Chair or past Cosgrove recipients may not be nominated. The SWF Chair term expires at the end of the SWSMC in which they complete their 2-year term. 5. All requested information must be completed and information submitted must be documented. Use “N/A” if the requested information is not applicable. 6. A narrative describing the candidate’s professional contribution to the Forum or ISM, locally or nationally, may be presented with the nomination form. The narrative should detail how the candidate’s volunteer contributions and leadership skills have advanced the Supply Management profession. 7.
